4Cs CT State Community College Dues Deduction "*" indicates required fields To be a member, you need to sign a membership form. The amount you pay is 1.2% of salary while you are on contract. But membership gives you a greater voice in decisions that affect your job. For example, only members can vote in union elections as well as create priorities for contract negotiations. Membership in the 4Cs is important because it gives you a voice in decisions that affect your job and other union affairs such as officer elections and contract ratification. It also enables the 4Cs to communicate with you through our publications and e-mail alerts about contract news and other relevant issues. By participating fully in the affairs of the union, you will become a supporter of improving the rights and benefits for all union members. Together we all become stronger. Member rights are set forth in the 4Cs Constitution available on our web site at www.the4cs.org/governance.First Name*Last Name*Street AddressStreet Address 2Zip Code*CityState/ProvincePlease select…AlabamaAlaskaAmerican SamoaArizonaArkansasCaliforniaColoradoConnecticutDelawareDistrict of ColumbiaFloridaGeorgiaGuamHawaiiIdahoIllinoisIndianaIowaKansasKentuckyLouisianaMaineMarylandMassachusettsMichiganMinnesotaMississippiMissouriMontanaNebraskaNevadaNew HampshireNew JerseyNew MexicoNew YorkNorth CarolinaNorth DakotaNorthern Mariana IslandsOhioOklahomaOregonPennsylvaniaPuerto RicoRhode IslandSouth CarolinaSouth DakotaTennesseeTexasUtahU.S. Virgin IslandsVermontVirginiaWashingtonWest VirginiaWisconsinWyomingArmed Forces AmericasArmed Forces EuropeArmed Forces PacificEmail* Home PhoneMobile PhoneBy providing my mobile phone number, I understand the Service Employees International Union, its local unions, and affiliates may use automated calling technologies and/or text message me on my cellular phone on a periodic basis. I hereby request and voluntarily authorize my employer to deduct from my earnings and to pay over to 4Cs an amount equal to the regular monthly dues uniformly applicable to members of 4Cs. This authorization shall remain in effect and shall be irrevocable unless I revoke it by sending written notice via U.S. mail to both the employer and 4Cs during the period not less than thirty (30) days and not more than forty-five (45) days before the annual anniversary date of this agreement or the date of termination of the applicable contract between the employer and 4Cs, whichever occurs sooner. This authorization shall remain in effect and shall be irrevocable unless I revoke it by sending written notice via U.S. mail to the 4Cs.
